I was able to answer half my question. It's an Amphenol 713-4270 and I can
get one from Allied Electronics for around $11.00. Now, does anyone have an
idea for crimping it?

Regards.

Can someone tell me the name of this plug and where I can get a new one
and
the appropriate crimper?

Regards.

Larry

Go to an electronics shop and ask to use the correct die and crimper. I've
tried alternatives and they do not last. If you can take the antenna to the
shop, that would best. Hopefully this will be free. Do not try it with the
wrong die - don't ask.
