I've used Spot for the past (I think) three or four years all over the Mediterranean and from Florida and the Bahamas to Maine and I give an A+. Although it's no replacement for an EPIRB, it's a good supplement for one and is a potential lifesaver.
In my experience, Spot does exactly what they advertise: gives me a quick and easy way to send an "All's well" to friends and family no matter where I am. I also link it to my boat website so anyone looking for us will know where the boat is. (Check it out at: www.bluewaternav.com if you're so inclined.) Small, easy to use, energy efficient (a set of lithium batteries lasts about a year), cost effective, and unfailingly accurate and reliable. Happily, I've never had occasion to use the 911 feature! No connection with Spot other than as a satisfied customer.

Any one use this and have any comments?
