I'd recommend Ronco to build your tank. They have hundreds to choose
from... I bet one will work for you. http://www.ronco-plastics.com/

I would try to have all the connections to the tank on top if possible.
I specified two discharge fittings on the top of my tank. They are fit
with dip tubes that go to the bottom of the tank. One leads to the deck
pumpout, and the other to the diaphragm pump. This eliminates the need
for a Y-Valve there (one less place to leak.) You didn't mention a
cleanout port. I would install at least a 4" one on the tank, so you can
have access if needed later.
Finally, I'd highly recommend Sealand's odorsafe hose. It's expensive,
but far better than any other. Do it right the first time and it'll save
headaches later.
