Opiates and opioids are a great deal more trouble than they're worth in a medical kit. They certainly are not life savers, as for instance antibiotics, or effective means to treat wounds or set bones. For pain relief  they need to be carefully titrated  and can be fatal if not used properly. Other painkillers are available that are almost as effective and are much safer, and are legal everywhere.

An effective painkiller for moderate to severe pain is Tramadol
Hydrochloride, which is not a controlled drug in most parts of the world..
Available both as a tablet and an injection, it is very effective.
