Klaus mentioned one way (custom RTCP).

Are you using handsets with a special key?  Otherwise, possibly you can assign standard keys and then use DTMF
detection in Asterisk.

Yes, of course - it depends if you just need any kind of self-made PTT
system or if you want to build PoC PTT.
