There was also a similar one for Washington State but I can't find it at the
moment. Perhaps someone closer has the form in PDF format or a website where
it is posted.
Noonsite, on its webpage has it referenced at the top

but no further references. I though I had it but can't find it offhand.
In either case it appears to have nothing to do with a cruising permit or
such.
